JOB DESCRIPTION

 

 

TITLE :  CARE MANAGEMENT CASE AIDE - MEALS ONLY

 

DEFINITION Reporting to a Care Manager or a Care Management Supervisor, this position is responsible for the care management of individuals who have declined OPTIONS or Waiver programs and are receiving home delivered meals only.  The care management aide provides services as specified and required by the PA Department of Aging for “Meals Only” consumers including annual reassessments, home visits, and telephone contacts.

 

QUALIFICATIONS :

Education and Experience:

  • Associate Degree (A.A.S) preferably in Gerontology or related field and minimum of 1 year experience in a human services agency. 

 

Professional Characteristics :

  • High energy level; able to manage a variety of tasks simultaneously.
  • Well-developed interpersonal and communication skills.
  • Well organized.
  • High level of flexibility.
  • Possesses advocacy skills and a sense of professional ethics.

 

Additional Requirements:

  • Must have a valid driver’s license, good driving record, and continuous access to a fully insured car.
  • Pre-employment physical.
  • Drug testing.

DUTIES AND FUNCTIONS:

  1. Performs all care management functions for “meals only” caseload.    
  1. Coducts NAT-E assessment of nutrition needs and eligibility for home delivered meals annually.
  1. Conducts semi-annual home visits and quarterly telephone calls to monitor eligibility, changing needs, and satisfaction with meals.
  1. Follows up on meal deliveries problems as needed.
  1. Completes all required documentation and data entry in SAMS and Oracle.

  • Meets PDA and PCA standards of care management practice.
  • Adheres to PDA consumer contact requirements.
  • Records and computer data entry are timely, accurate, and complete.
  • Ability to establish professional rapport with consumers, caregivers, providers, and PCA staff.
  • Assures quality services to consumers.
