In accordance with the Privacy Act ( 5 U.S.C.552 a), Freedom of Information Act, I expressly authorize any past or present employer, any law enforcement agency, federal, state or local, or any person who has personal knowledge of my character, work experience or criminal records to release this information to SRV, LLC (Go! Retail Group.)
I understand and acknowledge that, as a condition of being considered to be an Operator, I must submit to a credit history check and criminal background to be performed by a third party entity of Go! Retail Group choice. The purpose of these checks is purely commercial in nature, in furtherance of a potential commercial business relationship between Go! Retail Group and me. I understand Go! Retail Group may use the results of the credit history and criminal background check in determining whether I will become an Operator.
I understand that if I am approved to become an operator, Go! Retail Group may rely on this authorization and have additional background checks conducted during and throughout the term of my contract agreement without asking for my authorization again.
By signing below, I authorize the Company to contact any of my references listed, for purposes of inquiring about my qualifications for this opportunity. Release Form for Consumer Reports
