Position Responsibilities:
· Provides screenings and assessments of new patients for both Partial Hospitalization and outpatient services, both via telephone and in person.
· Devises, completes, and documents on a regular basis on the initial treatment plan, master treatment plan, integrated diagnostic summary, and treatment plan updates.
· Facilitates group therapy sessions and 1:1 sessions on a daily basis as needed.
· Establishes and maintains effective interpersonal relationships with the patient, work team, and all levels of the organization.
· Follows Patient Safety goals
· Completes daily tasks as assigned, completes all clinical documentation from group or individual therapy sessions on the day of service.
· Completes Biopsychosocial assessment within 48 hours of patient admission.
· Reports incidents to supervisor and Risk Management department within required timeframe in accordance with medical center policy.
Minimum Requirements:
· Licensed as an LMHC, LCSW, or LMFT in the State of Florida is required.
· Masters Degree in Mental Health Counseling, Social Work, or Marriage and Family
