The City of Virginia Beach Department of Human Services is seeking to hire a candidate for the ACT Clinician position who is highly ambitious and motivated with a passion to help others for the Assertive Community Treatment (ACT) program. The ideal candidate for these positions will be licensed as a LPC, LCSW, or license eligible, have experience providing community-based or in-home services to consumers with serious mental illness and or co-occurring disorders to include experience with providing skills training for activities of daily living, linking consumers to community resources, and providing crisis intervention and or hospital discharge planning.    

 

Duties:

  • Provide intensive community treatment and community-based services to consumers diagnosed with serious mental illness and/or co-occurring disorders. 
  • Provide comprehensive assessments for persons with chronic mental illness and co-occurring disorders to determine services and supports needed to promote safety and stability in the community.
  • Develop individualized treatment plans that respond to the needs of the person served and coordinates service delivery that coincides with the treatment plan. 
  • Provide linkage, monitoring, and coordination of services such as medical care, specialized therapy, residential and housing support, vocational readiness and placement, residential crisis stabilization, etc.  
  • Assist consumers with attending appointments with Social Security, Social Services, landlords, and other community support services appointments and meetings.
  • Serve as the primary care coordinator (SAI) for a caseload of 10-15 ACT Consumers.
  • Provide supportive counseling, outreach home visits, crisis intervention and advocacy.
  • Provide individual, group, family rehabilitative and supportive counseling, independent living training, and emergency/crisis intervention.
  • Assist in the provision of medical services, such as delivering and monitoring medications, scheduling for and transportation to psychiatric and medical appointments and requesting medical records. 
  • Assist with TDOs and is present at hearings for ACT Team consumers. Assists and participates in all hospital diversion, hospital in-reach, and hospital discharge and re-integration services.
  • Provide case and program consultation to individuals, organizations, agencies, groups, etc. that are involved with Virginia Beach ACT consumers.
  • Participate in team meetings, clinical case reviews and supervision. Provides mental health educational materials and programs to consumers and staff.
  • Maintain QA documentation standards in accordance to agency, DMAS and licensure regulations.
  • Provide emergency assessment, treatment and collaboration during work hours and via the ACT emergency phone rotation. 

Work ScheduleMonday - Friday, 8:00 am - 4:30 pm, one evening shift, 11:30 am - 8:00 pm, and alternating weekend day.

And here's the exciting part - the selected candidate will qualify for a $4,000 hiring bonus! 

Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0.

  • $1,000 will be awarded in the first paycheck.
  • $1,250 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.
  • $1,750 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.

 This position is also eligible to receive a $6,000 annual stipend to be paid in installments over the 26 pay periods.

Minimum Requirements:

MINIMUMS: Requires a Bachelors degree in a human services field (such as psychology, behavioral or mental health, special education, social work) and five (5) years of experience associated with such positions as clinician, counselor, therapist, social worker, psychologist or psychiatric nurse OR any combination of education (above the high school level) and/or experience equivalent to nine (9) years associated with such positions as clinician, counselor, therapist, social worker, psychologist or psychiatric nurse. Must have, or obtain, a current and valid DMV driver’s license.

SPECIAL REQUIREMENTS: Must be licensed with the Virginia Board of Counseling as a Qualified Mental Health Professional (QMHP), Licensed Professional Counselor (LPC), LSAT-P, LCSW, or a resident/supervisee in LPC or LCSW. All employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies.

Preferences:

Licensed as a LPC, LCSW, or licensed-eligible.
Community-based or in-home experience.
Two (2) or more years working with individuals with serious mental illness.
Experience with linking consumers to community resources.
Experience with evidenced-based clinical interventions.
Experience providing skills training for activities of daily living.
Experience providing crisis intervention and/or hospital discharge planning.
Strong experience with electronic health record.
Experience with an ACT team or experience working on multi-disciplinary team.
Experience providing medication assistance.

Special Instructions:

Must be licensed with the Virginia Board of Counseling as a Qualified Mental Health Professional (QMHP), Licensed Professional Counselor (LPC), LSAT-P, LCSW, or a resident/supervisee in LPC or LCSW.

VRS Contribution:   All full-time employees are required to contribute 5% of annual salary toward their retirement account; in accordance with VRS retirement provisions. This will be handled through a pre-tax payroll deduction.
