I certify that answers given herein are true and complete to the best of my knowledge.
I authorize investigation of all statements contained in this application for employment as may be necessary in arriving at an employment decision. I understand that all information obtained during my background investigation will be disclosed to Human Resources personnel, and individuals involved in making a hiring decision. I hereby release employers, schools, credit bureaus, third party background screening vendors or persons from all liability in responding to inquiries in connection with my application.
I understand that, after an offer of employment has been made to me, a medical examination and drug screening will be performed at the Company’s expense by a health professional of the Company’s choice. Thus, I agree to submit to such examinations and understand that any offer of employment and assignment to duties is contingent upon satisfactory completion of the examinations.
I also understand that in certain circumstances, it may not be possible to undergo the examinations or have the results of such examinations before I commence job duties. However, I acknowledge and agree that my continued employment and assignment to duties is contingent upon successful passing of such examinations.
I further acknowledge and agree that after employment, that I may be may be required to take medical examinations, where consistent with business necessity, to determine my fitness to perform job related functions. I understand that such examinations will be scheduled at reasonable times and intervals, and performed at the Company’s expense.
In consideration of my employment, I agree to conform to the rules and standards of the Company, as amended by the Company from time to time at its discretion. I further agree that my employment and compensation can be terminated at will, with or without cause, and with or without notice, at any time, either at my option or at the option of the Company. I understand that no employee or representative of the Company, other than its President, has the authority to enter into any agreement for employment for any specified period of time, or to make any agreement contrary to the foregoing. Further, the president of the Company may not alter the at-will nature of the employment relationship unless he/she signs a written document in which he/she specifically and clearly indicates the intent to do so.
I also understand that all offers of employment are contingent on the Company’s receipt of satisfactory results of background screening, including, but not limited to criminal, credit, employment, educational, worker’s compensation, civil, driving records checks and the provision of satisfactory proof of an applicant’s identity and legal authority to work in the United States.
I understand that any false, misleading or material omission of information in my application, interview(s) or pre-employment process may result in my disqualification of candidacy or in the event of employment, of employment termination.
