DISCLOSURE OF HEALTH LIMITATIONS

You are not required to disclose information about physical or mental limitations that you believe will not interfere with your job performance. CERTIFICATION
A. It is the policy of Punahou School to hire only U.S. citizens, aliens who are authorized to work in this country, and minors who are cleared to work by the Hawaii Child Labor Office. As a condition of employment, you will be required to produce original documents establishing your identity and authorization to work, and to complete the U.S. Immigration and Naturalization Services form I-9.

I understand I must be at least 14 years old to work at Punahou School.

B. I certify that the information contained in this application is true and correct to the best of my knowledge, and understand that any false or misleading statements or omissions, whenever discovered, regarding this application are grounds for disqualification from further consideration or for dismissal from employment at Punahou.
C. If employed by Punahou, I agree to conform to the guidelines and policies of Punahou, and understand that my employment at Punahou may be terminated at any time if the President or designee determines my job performance or conduct on the job is not satisfactory or is prejudicial to the interests of the school.
D. I authorize Punahou to verify all references and information provided by me in this application and release Punahou, any person or company responding to any reference or information from any claim or liability regarding any information or opinion supplied. I understand that any offer of employment is subject to satisfactory references.
E. I understand that Punahou may inquire into and consider any criminal conviction record that I may have after it makes a conditional offer of employment to me. I also understand that Punahou may withdraw a conditional employment offer if it determines that I have a criminal conviction record which bears a rational relationship to the duties and responsibilities of any position for which I am applying.
F. I understand and agree that all of the foregoing terms and conditions will become part of my employment agreement with Punahou if I am employed at Punahou.
