  2. I understand that any misrepresentation, falsification, or omission of information may disqualify me from further consideration for employment or, if discovered after hire, may result in termination of employment. 

  3. I understand that submission of this application does not guarantee an offer of employment and does not create a contract of employment. 

  4. I acknowledge that any offer of employment is contingent upon the successful completion of the Company’s required pre-employment processes, which may include background checks, drug screening, and/or verification of credentials as applicable to the position. 

  5. I understand that the Company reserves the right to withdraw or rescind any offer of employment if results from these processes do not meet Company or client-specific requirements. 

Background Check and Drug Screening Disclosure: 

Depending on the position and client requirements, the Company may require differing background checks and/or drug screening as part of the pre-employment or post hire requirements:

At-Will Employment Statement: 

Unless otherwise stated in a signed agreement authorized by the Company’s executive leadership: 

  1. I understand that employment with the Company is at-will, which means either I or the Company may terminate the employment relationship at any time, with or without cause, and with or without notice. 

  2. I further understand that no supervisor, manager, or representative of the Company has authority to make any agreement contrary to this at-will relationship unless it is in writing and signed by the Company’s authorized representative. 

Conditional Employment Acknowledgement: 

  1. I understand and acknowledge the following terms regarding conditional employment: 
    My employment with the Company is conditional and contingent upon the satisfactory completion of all required pre-employment processes, which may include, but are not limited to, background checks, drug screenings, employment verification, education verification (highest level achieved), and/or credential/license verification, as applicable to the position or client requirements. 

  2. If results from any of these processes do not meet Company or client-specific standards, or if information I provided is found to be false, incomplete, or misleading, the Company may rescind my offer of employment or terminate my employment immediately through the Adverse Action process. 

  3. I understand that beginning work prior to the completion of these checks does not alter the conditional nature of my employment. 

  4. I further understand that any hours worked during this conditional period will be paid in accordance with applicable laws; however, the Company is not obligated to continue employment if conditions are not met. The Company may be required to re-assign or remove me from an assignment, if specific client requirements are not met.  

  5. I acknowledge that this conditional status will remain in effect until the Company provides confirmation that all pre-employment requirements have been successfully satisfied.
