Qualifications: 

• Successful completion of National Certification as a Nurse Practitioner or of National Certification as a Physician Assistant.
• Must hold current licensure/certification as required by the State of Utah including a DEA number.
• Candidate must be a motivated, hardworking individual with the ability to multi-task.
• APC must possess strong interpersonal skills and excellent oral and written communication skills.
• APC must be able to work both as an individual and as a part of our amazing team.
• Previous Family Medicine training and background experience is preferred.

Description: The Family Practice Nurse Practitioner (NP) or Physician Assistant (PA) provides excellent care and treatment to our patients in our clinic under the direction of the physician. The NP/PA is accountable for providing optimal patient care through effective clinical practice, educating the patient and patient's family members, and other clinical staff. The NP/PA is responsible for patient assessment, diagnosis and determining a plan to ensure proper and complete patient care.

Other responsibilities could include:

• Ordering, interpreting, evaluating diagnostic tests to identify and assess patients' health care condition and needs.
• Coordinating and providing preventative care according to guidelines of our ACO

Hours: Patient Care hours will be:

• Mondays 3:00 pm - 7:00 pm
• Fridays 8:00 am - 12:00 pm
• Saturdays 8:00 am - 3:00 pm
• Able to pick up other shifts as needed at MD's discretion.
