Young Child Feeding Practice (IYCF) Knowledge, Attitude & Practice (KAP) Survey in Damazine and Rosaries localities, in Blue Nile State, Republic of Sudan

  1. BACKGROUND:

The Relief International Sudan’s nutrition strategy is designed to ensure the provision of lifesaving nutrition services for acutely malnourished children, pregnant and lactating women in the IDPs camps and vulnerable host communities, to prevent poor nutritional outcome through promotion of optimal infant feeding practices, proper hygiene and sanitation and improved maternal nutrition, micronutrient supplementation and nutrition education on locally available foods. In its strategy, RI puts emphasis on capacity building of health care workers which is done in collaboration with the State Ministry of Health (SMoH) in all the supported OTP/ TSFP sites and Stabilization centers, in Blue Nile state.

Under nutrition programme, RI implements Outpatient Therapeutic Feeding programme (OTP) services for children with severe acute malnutrition (SAM), and Targeted Supplementary Feeding Programme (TSFP) for moderately malnourished children through fixed teams. In addition to these supports,  the nutrition programme implements Infant and Young Child Feeding (IYCF), which  aims  at influencing change of attitude and behavior through use of community counselors, running of care groups (Mother Support Groups) and capacity building of community resource persons to implement IYCF.  The key indicators for the project include Exclusive Breastfeeding Feeding (EBF) and dietary diversity which the KAP intend to capture.

As a follow-up of the efforts that is being implemented in Blue Nile, there is need to establish a monitoring mechanism through an assessment of knowledge, attitude and practices of the community with regards to infant and young child feeding practices.The planned assessment will seek to understand the Knowledge, Attitudes, and Practices (KAP) on IYCF in five areas (Alwahda Geissan, Shanfor, Al-Duema, Alwefag and Shin-Tsrab) of Blue Nile state, where RI is implementing the IYCF and CMAM integrated project. The purpose of the KAP survey is to establish indicators for Infant and Young Child Nutrition (IYCF) activities within catchment population of five health facilities supported by RI in Damazine and Roseries in Blue Nile State. Further, the assessment would make practical recommendations and provide a platform for reflection and learning. Additionally, the findings and recommendations would be crucial in informing future program design and provide guidance to Relief International Sudan program (RI), community, government State Ministry of Health and other stakeholders on the most suitable strategies to be adopted for the implementation of nutrition projects during the next phases of the implementation of the nutrition programme. It is for this purpose that RI intends to conduct the IYCF KAP survey to establish the level of achievement of the IYCF interventionsin the above mentioned supported localities of Blue Nile. 

2. SCOPE OF WORK:The Consultant will be responsible for reviewing existing project documents and reports to get the required information on the indicators. The Consultant will also review previous KAP surveys on IYCF in the same geographical areas to get parameters for sample size calculation, develop a protocol for the planned KAP survey and manage the assessment. This will allow a comprehensive analysis of the progress of the IYCF KAP survey, using both quantitative indicator estimations; program information over the years, and qualitative data to be collected. The KAP will cover key IYCF indicators in line with the guidelines of the Ministry of Health of the Republic of Sudan.

3. MAIN OBJECTIVES:  The main objective of this survey is to determine knowledge, attitude, and practice of infant and young child feeding practices in selected villages of Damazine and Roseries in Blue Nile State, Sudan. The study will also identify an implementation strategy that is feasible in terms of program quality and community involvement in order to scale up the most effective approach to promote better child feeding practices.

Specific objectives

  • To obtain quantitative data on infant and young child feeding practices using the WHO IYCF Indicators.
  • To assess the consumption of the different types of food for children agedfrom 6-23 months within the 24 hours prior to the survey, and henceestimate the food diversity within the last 24hours.
  • To assess the factors related to IYCF practices.
  • To make recommendations based on findings
  • To determine the effectiveness, relevance, and appropriateness of thecurrent SBCC strategy

4. METHODOLOGY

The consultant is expected to propose and an appropriate study design involving both quantitative and qualitative data collection entailing beneficiary household survey; Health Facility document reviews, focused group discussions, beneficiary, Key informant, and stakeholder interviews. The survey should combine both quantitative and qualitative methods.

Locations:

The survey is to be conducted in Damazine locality (Alwefag, Shin-Tsrab),  andRoseries locality (Alwahda Geissan, Shanfor and Al-Dueima) in Blue Nile State.

Study population:

Mothers with children aged 0-23 months and under 5 years in the household will be interviewed to estimate the infant and young child feeding practices in all selected villages.

Start date of the Survey:

The duration of the assignment is 23 working days after the signing of a contract. Days are inclusive of traveling, fieldwork and reporting. The field data collection is expected to start on the 17thApril 2022.

5. DATA COLLECTION:

The nutrition program has tools that have been used for KAP surveys. These tools will be used with modifications as necessary with approval of the relevant technical leads (FMoH, Nutrition Survey technical working group, RI Nutrition Technical Coordinator). Enumerators who will be supervised by the consultant will collect the data for this survey. RI staff will be responsible for coordination of the survey and making all the logistical arrangements for data collection. All data will be collected and will be accessed by both RI focal person and consultant on real-time basis.

6. SPECIFIC TASK FOR THE CONSULTANT:

  • Develop and submit inception report (Protocol) which will include an interpretation of the tasks and design methodologies, which will be used in the KAP study. 
  • Develop appropriate tools and instruments for data gathering and present the original copy to the contact person in the RI MEAL department and at the FMoH.
  • Present KAP methodology to the Sudan Nutrition cluster Technical working group for validation.
  • Conduct a desk review (nutrition Survey reports, Baseline report and other KAP surveys) relevant to the survey.
  •  Conduct KAP survey: Train data enumerators and ensure quality of data being collected on daily basis.  
  • Undertake KII and FGD with key stakeholders in the field. 
  • Process and analyze the data, submit the draft report and present the findings at the State nutrition cluster.
  • Revise the report and based on the Nutrition Technical working and internal RI technical feedback, submit a final report to the nutrition and MEAL department of RI. 
  • Develop context based actionable recommendations which include specific recommendations based on evidence at state level. 

7. CONSULTANCY DELIVERABLES:

  • Inception report that includes the design, implementation, and supervision strategy of the KAP survey including data collection tools.
  • COVID 19 adherence plan.
  • Data Collectors training PowerPoint.
  • First draft KAP survey report including raw data set on approved platform as repository.
  • All instruments used for data collection
  • Submit PowerPoint presentation of the key findings to the Nutrition technical working Group
  • Final detailed report with recommendations

8. RI’s RESPONSIBILITY DURING KAP SURVEY  

 RI will provide logistical support to the consultant in the country. RI will facilitate logistical arrangement including travel (flight ticket) and accommodation in RI Guesthouse with the exception of meals. At the field level, RI staff will be responsible for survey and making all the logistical arrangements for data collection. All data will be collected and will be accessed by both RI focal person and consultant on real-time basis. This entails recruitment of enumerators, supervisors and mobilize the target groups for interviews. RI will provide input to the inception report, tools and KAP draft report.

PROFILE REQUIRED

  • Bachelor Degree in Human Nutrition or Bachelor Degree in Nutrition and Dietetics, Master’s Degree in Nutrition Science or Public Health. Post Graduate Diploma in Statistics or Biostatistics will be an added advantage.
  • Vast experience in Nutrition Survey (SMART survey and SQUEAC).
  • At least five years of work experience at national or international levels in infant and young child feeding and nutrition.
  • Proven experience in conducting IYCF-E surveys in Sudan and understanding thedynamics of  the context of Sudan.
  • Sound understanding of the international standards on IYCF-E and Experience in design and implementation of IYCF-E programs.
  • Experience in design and implementation of IYCF-E programs.
  • Experience in organizing and leading IYCF-E training
  • Experience and knowledge of IYCF data collection, analysis using EPI Info, SPSS and KOBO.

Knowledge of English language (including written English and reporting).  Ability to make clear presentations and disseminate study findings to both technical and non-technical audience.
